Delays are common, and they usually come down to four structural problems rather than any single oversight.
First, no single test settles the question. As MedlinePlus puts it plainly, there is usually not one specific test that shows whether you have a particular autoimmune disease. Diagnosis is assembled from symptoms, physical examination, several blood tests and sometimes imaging or biopsy.

The antinuclear antibody test
The ANA test looks for antibodies aimed at structures inside the cell nucleus. It is the standard entry point when several body systems seem involved at once. The ENA panel and other specific antibodies

What a positive result does and does not mean
This is the single most misunderstood part of the whole process, and it deserves stating flatly: a positive ANA is not a diagnosis of anything.
A 2025 review of ANA interpretation published in the Journal of Clinical Medicine makes the point directly. Positive ANA results occur in people who are entirely healthy, and low titers frequently carry no diagnostic significance at all. The review notes that titers above 1:160 tend to be more useful for separating genuine positives from the background positives found in healthy individuals, and that while certain staining patterns are linked to particular diseases, many patterns have no strong clinical correlation. Its conclusion for clinicians is that ANA results must always be read alongside the clinical picture, precisely to avoid misdiagnosis and unnecessary treatment.
The reverse error matters too. A negative ANA does not close the file. Rheumatoid arthritis, celiac disease, autoimmune thyroid disease and inflammatory bowel disease are all diagnosed through other routes entirely.
The practical consequence: an autoantibody result is one input among several. If a test comes back flagged, the useful next step is a conversation about what it means in your specific context, not a search for the disease name attached to the antibody. When to see a doctor
Book a routine appointment if you have had, for six weeks or longer, any of the following: fatigue that rest does not relieve, joint pain and morning stiffness lasting more than half an hour, recurring rashes, repeated low-grade fevers with no infection, persistent dry eyes and dry mouth, or unexplained weight change.
Seek medical care promptly, rather than waiting, for any of these:
- New numbness, weakness on one side, or sudden vision loss or double vision
- Shortness of breath, chest pain, or a cough that will not settle
- Swelling of the face, legs or ankles, or urine that looks foamy or bloody
- A fever above 101°F that persists, especially while taking medication that suppresses the immune system
- Severe abdominal pain, blood in the stool, or ongoing vomiting
- A rapidly spreading rash, or blisters and sores in the mouth
If you already have a diagnosis and take immune-suppressing medication, any new fever should be reported to your care team rather than waited out, because the usual signs of infection can be blunted.
How autoimmune conditions are treated
There is no cure for most autoimmune diseases, as NIAID states directly. The realistic goal is control: calming the immune attack, protecting the organ involved, and keeping the person in remission for as long as possible. Treatment falls into four broad categories.
Anti-inflammatory medication, including corticosteroids, is used to bring a flare under control quickly. Because of side effects with prolonged use, the aim is generally the lowest effective exposure for the shortest sensible period.

Making ANA results mean the same thing everywhere
A 2024 review in Autoimmunity Reviews described ten years of work by an international group dedicated to standardizing how ANA patterns are read and reported across laboratories worldwide. The group has classified new staining patterns and is building shared educational material for the laboratories that perform the test. What this means for you: the ANA report is a visual interpretation made by a human at a microscope, and this work is steadily reducing the variation between one laboratory and another — so a pattern described in one clinic increasingly means the same thing in another.
Combining joint antibodies instead of choosing between them
A 2023 systematic review and meta-analysis in RMD Open — meaning researchers pooled the results of many separate studies, in this case 36 of them — examined the different forms of rheumatoid factor. It found that testing rheumatoid factor broadly catches the most cases, while one particular form of it is the most specific, and that adding rheumatoid factor to anti-CCP testing improves diagnostic accuracy over either test alone. A separate 2025 study in Clinical and Experimental Medicine compared three joint antibodies head to head and found anti-CCP the most specific of them, with a newer marker, anti-MCV, showing promise as a complement rather than a replacement. What this means for you: if your clinician orders two or three overlapping antibody tests for the same joint problem, that is deliberate — the combination is more informative than any single result. Both remain research findings that inform practice rather than fixed rules.
Resetting the immune system in severe, treatment-resistant disease
A 2024 review in Rheumatology described the first uses of CAR T-cell therapy — a technique developed for blood cancers, in which a patient’s own immune cells are re-engineered in a laboratory and returned to the body — in people with severe lupus, systemic sclerosis and inflammatory muscle disease who had not responded to existing treatment. The early reports describe deep and lasting responses, some patients remaining well without ongoing medication. What this means for you: this is genuinely promising but still early. It involves very small numbers of patients with severe, treatment-resistant disease, and the authors are explicit that large randomized trials are needed before anything can be concluded about how safe and durable it is.
