Nausea at night usually has a mechanical explanation. When you lie flat, gravity stops helping your stomach keep its contents where they belong, and a stomach that felt merely full while you were sitting up can start to feel genuinely sick within minutes.

But the same symptom can also come from pregnancy, evening medicines, migraine, the inner ear, overnight blood sugar swings, and occasionally from something that needs treating tonight.

Nausea can be an atypical presentation of a heart attack. Women and people with diabetes more often have a heart attack without classic crushing chest pain, so nausea may be one of the loudest signals they get.

Call 911 and ask for an ambulance. Do not drive yourself. Also seek emergency care for vomiting blood or coffee-ground material, black tarry stools, severe abdominal pain, or inability to keep fluid down.

Why lying down changes how your stomach feels

Standing and sitting are quietly protective. Upright, gravity pulls your stomach contents away from the valve at the top, and swallowed saliva keeps washing the food pipe clean. Lying down removes both advantages. Stomach contents now sit level with the valve rather than below it, and anything that escapes upward stays in contact with the esophagus for longer, because you swallow far less while asleep.

Awareness shifts too. With nothing competing for attention, queasiness you would have ignored at 4pm becomes all you can think about at midnight.

Timing is your most useful clue. Nausea that begins within an hour of lying down and eases when you sit up points to position. Nausea that ignores position is more likely about hormones, medicines, blood sugar or nerves.

Reflux: the commonest explanation for nausea at night

Gastroesophageal reflux disease, usually shortened to GERD, is the single most frequent reason adults feel sick at night. The National Institute of Diabetes and Digestive and Kidney Diseases describes GERD as developing when the lower esophageal sphincter, the ring of muscle at the bottom of the food pipe, weakens or relaxes when it should stay shut.

What nighttime reflux actually feels like

Many people expect burning and get nausea instead. Reflux at night can present as a sour taste at the back of the throat, a lump-in-the-throat sensation, a dry cough that appears once you lie down, morning hoarseness, or plain queasiness with no burning. What clinicians commonly suggest

The measures clinicians most often discuss are unglamorous but well established. NIDDK advises eating meals two to three hours before lying down, and weight loss for those carrying excess weight. Raising the head of the bed a few inches, using blocks under the bedposts or a wedge rather than stacked pillows, keeps the torso on a slope instead of folding you at the waist. Many also suggest the left side rather than the right, and cutting evening alcohol.

Slow stomach emptying, late meals, and alcohol

Delayed gastric emptying and gastroparesis

The second mechanical explanation is a stomach still working when you go to bed. Gastroparesis means the stomach empties more slowly than it should without any physical blockage. NIDDK lists its symptoms as feeling full soon after starting a meal, feeling full long after eating, nausea, vomiting, bloating, belching, upper abdominal pain, heartburn and poor appetite. Diabetes is the most common known cause, through damage to the vagus nerve that drives the stomach muscle, though often no cause is found.

Fat slows stomach emptying in everyone, which is why a late takeaway is far more likely to produce nausea at 1am than the same calories at lunchtime. NIDDK also notes that opioid painkillers, some antidepressants and some bladder medicines delay gastric emptying.

Eating too close to bedtime, and alcohol

Alcohol is a double problem. It relaxes the valve at the top of the stomach and irritates the lining, so a nightcap taken to help with sleep is a reliable way to wake up nauseated. Fizzy drinks, chocolate, mint and large portions push the same way.

Medicines taken at night that commonly cause nausea

Look at your evening routine before you look for a disease. Many everyday medicines list nausea among their common effects, and taking them last thing means the peak level arrives while you are horizontal.

  • Antibiotics, particularly those taken on an empty stomach
  • Metformin, especially in the first weeks or after a dose increase
  • Opioid painkillers, which cause nausea directly and also slow the stomach
  • SSRI and SNRI antidepressants, most often early in treatment
  • Iron tablets, notorious for evening queasiness
  • Non-steroidal anti-inflammatory painkillers, which irritate the stomach lining
  • GLP-1 medicines used for diabetes and weight

The move is not to stop anything on your own. Write down what you take, when, and whether the nausea began within weeks of a new prescription or dose change, then bring that list to your prescriber. Timing, food and formulation can often be adjusted.

Pregnancy, anxiety, and migraine

Pregnancy: “morning sickness” is a misnomer

Say this plainly, because the folklore causes real distress: nausea in pregnancy is not confined to mornings. It can occur at any hour, and for many people it is worst in the evening and at night. Feeling fine at breakfast and sick every night does not mean you are not pregnant.

Blood sugar, sleep apnea, and other medical causes

Blood sugar swings are a common nocturnal cause in people with diabetes. Nocturnal hypoglycemia, a low blood glucose overnight, can present as nausea, sweating, shakiness, vivid dreams or a headache on waking. Very high glucose can also cause nausea. Nausea that is new and comes with chest discomfort, breathlessness, cold sweating, jaw, arm or back pain, or a sense that something is badly wrong, is a 911 call. Clinicians assess this with an ECG and blood tests including troponin, the cardiac marker, in an emergency department, not at home.

Raised pressure inside the skull

Nausea and vomiting driven by raised intracranial pressure have characteristic timing: worse in the early morning or on lying flat, and usually with headache. Visual disturbance, new weakness, confusion, unsteadiness or a headache that worsens on coughing or bending forward all raise concern. A sudden headache reaching maximum intensity within seconds, often described as the worst headache of a person’s life, is an emergency.

When to see a doctor, and what they may check

Book an appointment if nausea at night has lasted more than a couple of weeks, disrupts sleep several nights a week, stops you eating normally, or started within weeks of a new medicine. Also book if you are losing weight without trying, if swallowing is difficult or painful, or if you are over 55 with new upper digestive symptoms.

Seek care the same day for repeated vomiting, inability to keep fluids down, signs of dehydration such as dark urine, or severe abdominal pain. A clinician will start with your history, because timing and associated symptoms do most of the diagnostic work. Latest scientific advances in understanding nighttime nausea

Four recent pieces of work sharpen the picture of what happens overnight, and of how nausea gets interpreted.

El Hage Chehade and colleagues published a systematic review and meta-analysis in the Journal of Gastroenterology and Hepatology in 2023, pooling studies of obstructive sleep apnea and reflux disease. What was found: people with one condition were more likely to have the other, and the association held whichever diagnostic tools were used, and after accounting for sex, body weight, smoking and alcohol. What this means for you: if your nausea at night sits alongside snoring, witnessed pauses in breathing or daytime sleepiness, raise both problems at the same appointment.

Huang and colleagues, writing in Frontiers in Medicine in 2024, compared people with reflux disease who slept badly against those who slept well. What was found: the poor sleepers had more acid exposure while lying down, longer reflux episodes, more visible inflammation of the esophagus at endoscopy, and higher anxiety and depression scores. What this means for you: nighttime reflux and disturbed sleep reinforce each other, so a plan addressing only the acid, or only the sleep, tends to underdeliver.

Staller and colleagues produced the American Gastroenterological Association Clinical Practice Guideline on Management of Gastroparesis, in Gastroenterology in 2025. What was found: the panel recommended a four-hour gastric emptying study rather than a shorter two-hour version when delayed emptying is suspected. What this means for you: it is reasonable to ask which protocol was used, because a short test can miss the diagnosis.

Mousavi and colleagues reviewed sex differences in acute coronary syndrome in Wiener klinische Wochenschrift in 2023. What was found: women more often presented with symptoms that did not look obviously cardiac, which contributed to longer delays between symptom onset and treatment, and women were less likely to receive guideline-recommended therapy. I am pregnant and feel sick every evening. Is that normal?

Yes, and the phrase “morning sickness” is a misnomer that misleads a lot of people. Nausea and vomiting of pregnancy can happen at any hour, and for many people it is worst in the evening or overnight. Feeling well at breakfast and dreadful at 9pm is a recognized pattern. What is not something to simply endure is severe, persistent vomiting with weight loss and dehydration, known as hyperemesis gravidarum. That needs medical care, sometimes including fluids given into a vein. Contact your maternity team if you cannot keep fluids down.

How do I stop nausea fast at night?

In the moment, sit up rather than lying flat, and stay upright for a while before trying to sleep again. Sip small amounts of cool water rather than gulping. Cool, still air on your face helps some people, as does slow breathing through the nose. Avoid strong smells. For future nights, the measures clinicians most commonly suggest are leaving two to three hours between your last meal and bed, keeping evening meals smaller and lower in fat, skipping alcohol, and raising the head of the bed on blocks. Any medicine for nausea should come from your own clinician.

Could this be my heart?

It can be, and this is the one question not to sit on. Nausea is a recognized presentation of a heart attack, and it is more likely to be the leading symptom in women and in people with diabetes, who more often have a heart attack without classic chest pain. If your nausea is new and comes with chest discomfort, breathlessness, cold sweating, light-headedness, or pain in the jaw, neck, shoulder, arm or back, call 911 now. Do not drive yourself, do not wait until morning, and do not talk yourself out of it because the chest pain seems mild.

What if I have no reflux, no pregnancy and no obvious cause?

Unexplained nausea at night still has a workable list behind it. Common possibilities include medicines you no longer think of as new, anxiety, migraine without prominent head pain, inner ear causes triggered by rolling over, obstructive sleep apnea, gallbladder disease, thyroid problems, and blood sugar swings. Keeping a two-week diary of what you ate, when you went to bed, what you took, and exactly when the nausea started will usually narrow this faster than any single test.
