Low globulin levels turn up on routine blood work far more often than most people expect, and for the majority they are not a sign of disease. The single most useful thing to know is that globulin is usually not measured at all: your laboratory calculates it by subtracting albumin from total protein, so the result carries the measurement error of both. In this article you’ll learn where that number comes from, why a mildly low value alongside a normal albumin and no symptoms is often of little consequence, and the one pattern that genuinely deserves attention: a persistently low globulin in someone who keeps getting infections. Both halves of that message matter, and this page is written to give you each of them plainly.

What globulin is, and why the number on your panel is calculated

Globulin is not a single substance. It is a whole family of blood proteins grouped together by how they behave in the laboratory. The family includes transport proteins that carry hormones, iron and copper around the body, clotting factors, complement proteins, and the immunoglobulins (antibodies) that your immune system uses to recognise and neutralise germs.

Here is the part that most articles skip. On a standard comprehensive metabolic panel, no instrument measures globulin directly. The laboratory measures proteine totale and it measures albumină. It then subtracts one from the other and reports the difference as globulin. That is the whole calculation.

The consequence is important. A calculated value inherits the imprecision of both measurements it is built from. Small day-to-day variation in total protein, slight differences between analysers, how long the tourniquet stayed on your arm, whether you were lying down or sitting, how well hydrated you were, even the method the laboratory uses to measure albumin, can all nudge the subtraction by a small amount. A globulin sitting a little below the reference range is therefore, quite often, arithmetic rather than pathology.

This does not mean the number is useless. It means it should be read as a signpost rather than a diagnosis. A genuinely low globulin points in one of two broad directions: protein is being lost from the body, or fewer antibodies are being made. Sorting between those two possibilities is what the rest of the work-up is for.

What a low globulin usually means, and when it means very little

Most people who search for this topic have one mildly low figure on an otherwise unremarkable panel, feel perfectly well, and are understandably worried. For that situation the honest answer is reassuring: an isolated, mildly low globulin with a normal albumin, normal total protein and no symptoms is rarely the first sign of a serious disease. It is far more commonly a reflection of the calculation, of normal biological variation, or of a laboratory reference range that does not fit you particularly well.

Reference ranges are built from the middle 95 per cent of a reference population, which means roughly one healthy person in twenty will sit outside them on any given test through no fault of their own. Add a value that is derived by subtraction, and marginally low results become common.

What changes the picture is context: how low the value is, whether it is falling on repeated tests, what albumin is doing alongside it, whether you have symptoms, and what medications you take. The table below sets out the four patterns that account for almost everything seen in practice.

The albumin/globulin ratio: the same finding wearing a different name

Many people arrive at this subject not through the globulin line itself but through the A/G ratio printed just beneath it. The raportul albumină/globulină is simply albumin divided by globulin, and because globulin sits on the bottom of that fraction, a low globulin automatically produces a high A/G ratio. They are two ways of describing one result.

This matters because a high A/G ratio often looks more dramatic on a report than a slightly low globulin does, and it is easy to read it as a separate abnormality requiring a separate explanation. It is not. If your globulin is low and your albumin is normal, a raised A/G ratio is the arithmetic consequence, and it should be interpreted exactly as the low globulin is interpreted.

The opposite finding, a low A/G ratio, is driven by the globulin fraction rising rather than falling, and belongs to a different discussion entirely. Losing protein: the kidney, the gut, the liver and the skin

The first direction a genuinely low globulin can point is loss. If protein is leaving the bloodstream faster than the body can replace it, both albumin and globulin fall together. That is the signature to look for: a low globulin accompanied by a low albumin is a very different result from a low globulin on its own.

Kidney disease and nephrotic syndrome

Healthy kidneys keep protein in the blood. When the filtering units are damaged, protein leaks into the urine. In nephrotic syndrome this loss is heavy enough to cause a low blood albumin, swelling of the legs, ankles or the tissue around the eyes, and urine that looks persistently frothy. Because immunoglobulins are lost along with albumin, this is one route by which protein loss and reduced immune defence overlap. The gut: protein-losing enteropathy and malabsorption

Protein can also be lost through the intestinal wall. Protein-losing enteropathy is not a single disease but a mechanism, seen in inflammatory bowel disease, lymphatic obstruction, certain infections and some conditions affecting the heart. Coeliac disease belongs to a related group in which the intestine is damaged and absorption is impaired rather than protein simply leaking out. Chronic diarrhoea, unintended weight loss and swelling are the clues here, and coeliac serology is often part of the work-up. Severe malnutrition, whether from illness, poor intake or malabsorption, reduces the raw material available for protein synthesis and lowers both fractions.

The liver and extensive burns

The liver manufactures albumin and a substantial proportion of non-antibody globulins. In advanced liver disease, synthesis falls, and albumina scăzută is usually the more visible finding. Extensive burns cause protein loss on a large scale through damaged skin, and are managed in hospital where protein levels are monitored as part of overall care.

Making fewer antibodies: primary and secondary causes

The second direction a low globulin can point is reduced production of immunoglobulins. Because antibodies make up a large share of the globulin fraction, a fall in immunoglobulin production can pull the calculated globulin down even when albumin is entirely normal. This combination, a low globulin with a normal albumin in someone who is unwell with infections, is the one that changes management.

Primary antibody deficiency

Primary immunodeficiencies are disorders of the immune system itself rather than the consequence of another illness. Contrary to the common assumption that these are childhood conditions, several are most often recognised in adults. Common variable immunodeficiency (CVID) is the most frequent symptomatic form seen in clinical medicine and is characterised by low IgG together with low IgA and/or IgM and a poor antibody response to infection or vaccination. Selective IgA deficiency is the most common primary antibody defect of all; many people with it have no symptoms at all, while others have recurrent respiratory or gut infections.

Far more people have a low immunoglobulin level because of something else happening in their body or because of a medication. The important secondary causes include rituximab and other B-cell-depleting therapies used in rheumatology, neurology and haematology; long-term corticosteroids; chemotherapy; some antiepileptic drugs; heavy protein loss through the kidney or gut; and blood cancers such as chronic lymphocytic leukaemia and myeloma, in which normal antibody production is suppressed. None of these is a reason to change or stop a prescribed treatment on your own initiative. They are a reason for the team who prescribed it to monitor immunoglobulins and act on the result.

The pattern that matters most: low globulin plus recurrent infections

If you take one clinical message from this page, take this one. A persistently low globulin in someone who keeps getting infections should prompt direct measurement of immunoglobulin levels: Anticorpii, IgA and IgM. Not a repeat calculated globulin. The actual antibody levels.

The infection history that should trigger this is specific and recognisable. Infections needing antibiotics several times a year. Repeated sinus infections sau infecțiile urechii in an adult. A chest infection that clears only partly, or returns as soon as the antibiotics finish. A diagnosis of bronchiectasis. Infections that are unusually severe, unusually prolonged, or caused by organisms that do not normally trouble healthy people.

The reason this matters so much is timing. Primary antibody deficiency is characteristically diagnosed years after the first meaningful symptom, because recurrent chest and sinus infections are common in the general population and are treated one at a time rather than being recognised as a pattern. During those years, repeated lower respiratory infections can cause permanent, irreversible widening and scarring of the airways. Once bronchiectasis is established it does not reverse. Immunoglobulin replacement started earlier reduces infection frequency and protects lung function; started later, it cannot undo damage already done.

That is why a low globulin, on its own a fairly weak signal, becomes a genuinely useful one when it sits next to an infection history. It costs very little to measure immunoglobulins. The cost of not measuring them, in the small minority who need it, is measured in lung function.

Red flags that warrant medical review

See a doctor if you have a low globulin together with any of the following

  • Infections that need antibiotics several times a year
  • A chest infection that keeps coming back, or never fully clears
  • Unexplained swelling of the legs or around the eyes, particularly with frothy urine
  • Unintentional weight loss with persistent diarrhoea
  • A low globulin while taking rituximab, another B-cell-depleting drug, long-term steroids or chemotherapy

None of these is an emergency in itself, but each is a reason to have the result properly evaluated rather than filed away.

What your doctor may do next

Investigation is stepwise, and for most people it stops at the first step.

The first move is usually to repeat the test, with total protein and albumin measured properly, to establish whether the low value is real and persistent or a one-off. A single borderline result is rarely acted on.

If albumin is low as well, attention turns to loss and to synthesis: urine protein testing, kidney function, liver function, and questions about bowel symptoms and nutrition. If albumin is normal and the globulin is genuinely low, attention turns to antibody production, and immunoglobulin levels are measured directly.

Treatment, where any is needed, is always treatment of the cause. There is no medication that raises globulin as an end in itself, and no reason to want one. Where a significant antibody deficiency is confirmed, immunoglobulin replacement therapy may be offered under specialist supervision.

Latest scientific advances in low globulin and immune testing

A 2025 clinical review of common variable immunodeficiency in Annals of Allergy, Asthma & Immunology restates the diagnostic problem clearly: CVID is the most frequent symptomatic primary immune defect seen in practice, most people are diagnosed in early to middle adulthood rather than childhood, and a delay of several years between the first characteristic symptom and diagnosis is the norm in every country where it has been studied. What this means for you is that a doctor considering antibody deficiency in an adult with recurrent infections is following current thinking, not overreacting.

A 2025 nationwide multicentre study in Frontiers in Immunology described a cohort of people with CVID and found that lung infections were by far the most common problem, and that bronchiectasis, the permanent airway widening that follows repeated chest infections, was already present in a substantial proportion of patients at the time they were finally diagnosed. What this means for you is that the value of testing early is not theoretical; the damage accumulates during the delay.

A 2023 review in the same allergy journal examined hypogammaglobulinemia (low immunoglobulins) and infections following rituximab. It concluded that systematic checking of immunoglobulin levels and B-cell counts, both before treatment and periodically afterwards, helps identify the minority of patients whose antibody levels do not recover and who may benefit from vaccination, preventive antibiotics or immunoglobulin replacement. What this means for you is that if you are on a B-cell-depleting drug, a low globulin is information your specialist team can act on, not a reason to stop treatment.

Is a low globulin serious?

Usually not, on its own. A mildly low globulin with a normal albumin, no symptoms and no relevant medication is more often a quirk of a calculated value than a sign of disease, and it is frequently normal on repeat testing. The exception is specific and worth knowing: a globulin that is persistently low in someone with recurrent, lingering or unusually severe infections should lead to direct measurement of immunoglobulins, because an antibody deficiency found early can be treated before it damages the lungs. Both statements are true at the same time, and neither cancels the other out.

Can I raise my globulin level with diet or protein supplements?

No, and this framing is actively misleading. A low globulin is not a dietary deficiency and eating more protein or taking protein powders will not correct it. Globulins are made by the liver and by immune cells in response to biological signals, not in proportion to protein on your plate. Where malnutrition or malabsorption is genuinely the cause, the treatment is medical management of the underlying condition, not a higher protein intake chosen by yourself. Advice to eat more protein to fix a low globulin distracts from the questions that actually matter: is protein being lost, and are antibodies being made.

What does a high albumin/globulin ratio mean?

It is the mirror image of a low globulin, not a separate finding. Because the ratio is albumin divided by globulin, anything that lowers the globulin figure raises the ratio automatically. If your albumin is normal and your globulin is slightly low, the raised ratio carries exactly the same weight as the low globulin does, which in most cases is very little. It is interpreted using the same questions: is albumin normal, are there symptoms, are there recurrent infections, and are you on any treatment that suppresses antibody production.

Should the test be repeated, and how soon?

In most cases yes, and there is rarely any urgency. Repeating total protein and albumin after a few weeks to a few months distinguishes a transient or borderline value from a persistent one, and a persistent result is far more meaningful than a single reading. If you have symptoms, an infection history, or you are taking a treatment known to lower antibody levels, your doctor may skip the wait and measure immunoglobulins straight away. Your own clinician will set the interval based on the whole picture.

Can a low globulin simply be a laboratory error?

It can certainly be an artefact, which is not quite the same thing. Because globulin is derived by subtracting albumin from total protein, anything that shifts either measurement shifts the result. Dilution from intravenous fluids, prolonged tourniquet time, posture during the draw, sample handling, and differences in the albumin method between laboratories can all move the figure slightly. This is precisely why repeating the test, ideally at the same laboratory, is the sensible first response to an unexpected mildly low value.

Which specialist deals with a low globulin?

It depends on the direction the results point. If protein loss through the kidneys is suspected, a nephrologist. If bowel disease or malabsorption is the likely route, a gastroenterologist. If immunoglobulins are confirmed low and you have a history of recurrent infections, a clinical immunologist is the relevant specialist, and this is the referral that should not be delayed. Your primary care doctor coordinates this and will usually complete the first round of testing before referring.
