A CA 15-3 blood test measures a protein that breast tissue sheds into the bloodstream. Doctors use it mainly to monitor breast cancer that has already been diagnosed and has spread, rather than to look for cancer in the first place. If your result came back above the reference range, it warrants a conversation with your doctor, but it is not a diagnosis on its own. Benign conditions such as liver disease, non-cancerous breast changes, pregnancy and breastfeeding raise this marker too, and they do so commonly.

What a CA 15-3 blood test actually measures

CA 15-3 stands for cancer antigen 15-3. Despite the name, it is not a substance that only cancer produces. The test detects circulating fragments of a large protein called MUC1, short for mucin 1.

MUC1 sits on the surface of cells that line glands and ducts throughout the body, including the milk ducts of the breast. Healthy breast cells produce it every day, and small amounts drift into the blood all the time. That is why almost everyone has a measurable CA 15-3 level, including people who have never had cancer.

Why the level can rise

When a large number of breast cancer cells are present, they often produce more MUC1 than normal cells and release it into the blood more readily. The more cancer tissue there is in the body, the more marker tends to appear in a blood sample. This is the logic behind the test: it is a rough gauge of how much disease is present, which makes it useful for tracking change over time.

That same logic explains the test’s biggest limitation. A small tumor sheds very little MUC1. So the marker is at its least informative exactly when cancer is at its earliest and most treatable stage, and at its most informative when disease is advanced.

How the result is reported

Results appear in units per milliliter, written U/mL. Most laboratories treat a value under roughly 30 U/mL as within their reference range, but that figure is not universal: different manufacturers make different testing kits, and each produces slightly different numbers from the same tube of blood. The range printed on your report belongs to the laboratory that ran your sample, and that is the one that applies to you. Why doctors order a CA 15-3 blood test

CA 15-3 has one well-established job and several it is not suited to. Understanding the difference removes a lot of unnecessary worry.

Monitoring advanced or metastatic breast cancer

The main use is following someone who already has a confirmed diagnosis of advanced or metastatic breast cancer, meaning cancer that has spread beyond the breast. During treatment, the level is measured every few weeks or months. A falling level, read alongside scans and how the person actually feels, suggests treatment is doing its job. A level that climbs steadily over several samples may prompt the oncology team to look more closely with imaging.

The marker never decides anything by itself: imaging and clinical examination carry more weight. Tumor markers work this way across the board, as you can see in our overview of tumor markers and their limits.

Why CA 15-3 is not a screening test

This is the point that matters most. CA 15-3 is not a blood test to check whether you have breast cancer, and it should never be presented as one.

The National Cancer Institute puts the reasoning plainly in its fact sheet on tumor markers: studies of circulating tumor markers have generally found that they do not work well for screening. They miss too many people who do have the disease, and they flag too many people who do not.

For CA 15-3 specifically, two things go wrong at once:

  • Most early breast cancers never raise it. A tumor small enough to be caught early usually does not shed enough MUC1 to move the number.
  • Many benign conditions do raise it. In a person with no symptoms, a mildly high CA 15-3 is far more likely to reflect something harmless than cancer.

Breast cancer is found through a different route entirely: mammography, clinical breast examination, and, when something looks suspicious, targeted imaging and biopsy. Mammography remains the primary screening tool, and no blood marker has replaced it.

Why guidance advises against routine testing after early-stage treatment

If you have been treated for early-stage breast cancer and are now in follow-up, you may wonder why nobody is checking your CA 15-3 every few months.

This is deliberate. The American Society of Clinical Oncology (ASCO) and the National Comprehensive Cancer Network (NCCN) both advise against routine tumor marker testing, CA 15-3 included, for surveillance of people treated for early-stage breast cancer who have no symptoms. The reason is evidence, not cost. Studies have not shown that picking up a recurrence this way helps people live longer or feel better. What it reliably does produce is anxiety, false alarms, and follow-up scans that turn out to be unnecessary.

Standard follow-up after early-stage treatment centers on asking about symptoms, physical examination, and regular mammography. If your team does order the marker, it is usually because something specific prompted the question, and that is a good thing to ask them about directly.

Benign causes of a raised CA 15-3

A raised CA 15-3 is not a cancer diagnosis. Because MUC1 is made by normal glandular tissue, anything that irritates, inflames or multiplies that tissue can push the number up. So can anything that slows the body’s clearance of the protein.

Common non-cancerous causes include:

  • Benign breast disease — fibroadenomas, simple cysts, fibrocystic changes and mastitis can all lift the level modestly.
  • Liver disease — hepatitis, fatty liver disease and cirrhosis are among the most frequent benign causes. The liver helps clear circulating proteins, and when it is under strain the marker accumulates.   • Lung conditions — sarcoidosis, tuberculosis and other chronic inflammatory lung diseases can raise it.
  • Ovarian and pelvic conditions — endometriosis and benign ovarian cysts are recognized causes.

There is a useful pattern here. Benign causes typically produce modest elevations that stay put or drift up and down. Cancer-related rises in known advanced disease tend to be larger and climb steadily across consecutive samples. This is a tendency, not a rule, which is why nobody interprets the number in isolation.

How to read your CA 15-3 result

One number tells you very little

A single CA 15-3 value, taken on its own, is close to uninterpretable. It cannot tell you whether the MUC1 in your blood came from a tumor, from an inflamed liver, from pregnancy, or from perfectly ordinary breast tissue. The test simply does not carry that information. Only your wider clinical picture does.

The trend on the same assay is what counts

What doctors actually watch is the direction of travel across several samples spaced over weeks or months. Is it flat? Drifting? Climbing consistently?

For that comparison to mean anything, samples need to be measured the same way. Kits are calibrated differently, so a result of 28 from one laboratory and 34 from another may represent identical amounts of protein. Switching laboratories mid-monitoring can create an apparent jump that is pure measurement artifact, so staying with one laboratory is worth the effort.

A rise during treatment is not always bad news

Early in chemotherapy, the marker sometimes climbs for a few weeks before falling. This is often called a spike or flare, and it is thought to reflect dying tumor cells releasing their contents into the blood. It can look alarming on a graph while actually signaling that treatment is working. Your oncology team expects this and reads it in context.

Why a normal CA 15-3 does not rule out breast cancer

This deserves its own section, because a normal result offers less reassurance than people assume.

Plenty of breast cancers never raise CA 15-3 at all. Most early-stage cancers do not, simply because they are too small. And even among people with metastatic disease, a meaningful proportion have tumors that produce little MUC1 or release little of it into blood. Their marker sits comfortably in the normal range while disease is present.

The practical consequence is straightforward. A normal CA 15-3 is not an all-clear, and it is not a substitute for mammography or for getting a symptom looked at. If you have found a lump, noticed skin or nipple changes, or have any new breast symptom that concerns you, that needs clinical assessment and imaging on its own merits. The blood marker does not change that pathway in either direction.

CA 15-3 and CA 27.29: what is the difference

If your report says CA 27.29 rather than CA 15-3, you have not been given a different or lesser test.

Both tests detect the same underlying protein, MUC1. They simply use different antibodies to grab hold of it, which is why they carry different names and produce different numbers. Their clinical meaning is regarded as essentially interchangeable, and no major guideline prefers one over the other. Laboratories choose one and stick with it.

The one practical point worth knowing is that the two are not interchangeable within your own monitoring. Because the numbers sit on different scales, a CA 15-3 of 32 and a CA 27.29 of 38 cannot be plotted on the same graph. If your care moves between hospitals and the marker changes, expect the baseline to reset rather than reading it as real change.

When to see a doctor

Most people reading this are looking at a number and trying to work out how worried to be. A calm rule of thumb: the marker is a reason for a conversation, never a reason for a conclusion.

It is worth booking an appointment to discuss your result if:

  • Your CA 15-3 is above the laboratory’s range and nobody has explained why the test was ordered.
  • The level has risen across two or more samples rather than remaining stable.
  • You have a new breast symptom — a lump, a change in skin texture, nipple discharge or retraction, or persistent one-sided pain — whatever the marker says.
  • You have a personal or family history of breast cancer and want to understand what your follow-up should actually involve.
  • You are being monitored for advanced disease and your result has changed in a way nobody has talked you through.

Seek prompt medical advice, rather than waiting for a routine appointment, if you notice a rapidly growing lump, skin dimpling or puckering, a nipple that has newly turned inward, or unexplained weight loss alongside persistent bone pain. These are reasons to be seen regardless of any blood result.

Your doctor will place the number in context: your history, your examination, your other blood work, and imaging where appropriate. That context is exactly what the number lacks on its own.

Latest scientific advances in CA 15-3 testing

Research on CA 15-3 has moved away from asking whether it can find cancer towards defining what it can contribute once cancer is known. The studies below were identified through PubMed; full references appear in Sources.

A normal marker does not exclude recurrence

A 2024 study in the Journal of Nuclear Medicine compared CA 15-3 levels with PET/CT scan findings in 154 people being assessed for breast cancer recurrence. It was a cross-sectional study — a snapshot of a group at one point in time. Among those whose CA 15-3 was raised, most did have recurrence on the scan, and higher levels tracked with a greater burden of spread.

The more important finding ran the other way. Most people who did have recurrence on their scan had a completely normal CA 15-3 — more than half of confirmed recurrences would have been missed had the marker been trusted as a gatekeeper. The authors concluded that a normal CA 15-3 does not exclude recurrence, and that imaging should still be considered when recurrence is suspected.

What this means for you: if you have symptoms that worry you or your team, a reassuring CA 15-3 is not a reason to skip a scan. The marker is a poor filter and a better tracker.

Where markers sit alongside newer blood tests

A 2026 review in Seminars in Oncology Nursing examined how traditional serum tumour markers, including CA 15-3, compare with newer technologies that read fragments of tumour DNA circulating in blood, drawing on current practice guidance including NCCN. The authors’ assessment was balanced: markers such as CA 15-3 remain a cornerstone of practice because they are cheap, widely available and useful for tracking treatment response over time. Their sensitivity and specificity, however, are too limited for early detection. Circulating tumour DNA, the review noted, can offer earlier signals of recurrence and information about which treatments a tumour may respond to.

What this means for you: CA 15-3 is not being abandoned, but it is increasingly understood as one instrument in a set rather than a stand-alone answer. If a newer blood test is suggested, it is likely to sit alongside the marker rather than replace it.

A rising marker as a trigger for imaging

A 2026 study in Bioinformation followed 50 people treated for breast cancer whose CA 15.3 had risen during follow-up but whose conventional imaging had found nothing. Whole-body PET-CT located hidden sites of recurrence, and the scan findings changed clinical management for about four in five of them, most often by directing radiotherapy or changing systemic treatment.

What this means for you: this study describes people already under specialist care with a documented rising trend, not people with a single unexplained result. Read carefully, it reinforces the same message: the marker’s value lies in prompting a closer look, whilst imaging does the actual finding.

Looking at more than one marker at a time

A 2025 study in La Tunisie Médicale measured CA 15.3 alongside a different marker called CYFRA 21.1 in 110 women with breast cancer receiving chemotherapy, compared with 79 people without cancer. The two markers correlated with each other, most closely in certain tumour subtypes and in women with metastatic spread. The researchers noted directly that ASCO does not recommend routine use of CA 15.3, and suggested that several markers together may give a fuller picture of treatment response than any single one.

What this means for you: this was a modest, single-country study, not a practice-changing trial, and it does not mean you should ask for extra markers. It illustrates a direction of travel: researchers are combining signals precisely because no individual marker, CA 15-3 included, is reliable enough alone.

Can I ask for a CA 15-3 blood test to check that I don’t have breast cancer?

You can ask, but most doctors will explain why it would not answer that question. CA 15-3 is not designed or validated for finding cancer in people without symptoms. A normal result would not mean you are clear, because most early breast cancers never raise it. A high result would most likely reflect something benign, while still causing real worry and probably triggering scans you did not need. If your concern is about detecting breast cancer early, the conversation worth having is about mammography, your personal risk factors, and family history. That pathway has evidence behind it. My CA 15-3 is slightly high but I feel completely well. What happens now?

A mild, isolated elevation in someone who feels well and has no cancer diagnosis is usually managed by understanding the likely cause and, if appropriate, repeating the test later to see whether it is stable. A single number slightly outside the range is not treated as an emergency.

What does a high CA 15-3 blood test mean if I have already been diagnosed?

It depends entirely on your situation and on the trend. If you have advanced disease and are on treatment, your team reads the direction across several samples rather than any one value, and always alongside your scans and symptoms. A rise may prompt closer imaging; it may also be a temporary flare early in chemotherapy. If you were treated for early-stage disease, the marker is not part of standard follow-up at all, so an unexpected result is worth asking about directly. Your oncology team is the right source for what your specific number means.

Does anything I eat or do change my CA 15-3 level?

No diet, supplement or exercise routine has been shown to lower CA 15-3, and you should be cautious of anything that claims otherwise. The level reflects how much MUC1 protein is circulating, which is driven by underlying biology — glandular tissue activity, inflammation, liver function, or disease burden — not by lifestyle choices. Looking after your general health is worthwhile for many good reasons, but shifting this particular number is not one of them. What matters more than frequency is consistency: measurements taken the same way, at the same laboratory, so the trend means something. If you are unsure why yours is being checked at a given interval, that is a fair and useful question to ask.

Can CA 15-3 stay above the range even after successful treatment?

Yes, and this is more common than people expect. Some individuals settle at a level that sits a little above the standard reference range and simply stays there. Clinicians sometimes describe this as a personal baseline. Once that pattern is established, what they watch is movement away from your own baseline rather than the printed range on the report. It can be unsettling to keep seeing a flagged value, but a stable number that has not moved in a year is telling a very different story from one that is climbing.
