Doctors order this test either to explain something or to monitor something. Who expert bodies say actually needs testing
Here the guidance has shifted. In its 2024 clinical practice guideline, the Endocrine Society suggests against routine 25-OH vitamin D testing in generally healthy people with no established medical reason for it, having found no clinical trial evidence that screening healthy adults improves their health.
The US Preventive Services Task Force reaches a related conclusion differently, stating that current evidence is insufficient to weigh the benefits and harms of screening adults without symptoms. “Insufficient evidence” does not mean screening is useless; it means the studies needed to settle the question have not been done.

This is why two people with the same number hear different things. Vitamin D’s best-established role is helping your intestine absorb calcium. When it runs low, less calcium gets in, and your body compensates by raising parathyroid hormone, which draws calcium from your bones to keep blood calcium steady. Blood calcium stays normal; bone quietly pays for it.
Sustained over years, this weakens the skeleton. Severe, prolonged deficiency causes osteomalacia in adults, meaning bone that is soft because it is inadequately mineralised, and rickets in children. Symptoms are often quiet: fatigue, aching bones, weakness climbing stairs or rising from a chair. Many people with low readings feel entirely well, which is why the number alone never tells the whole story.
High vitamin D: rare, and almost always from supplements
A high result is uncommon. Sunshine will not cause it, because your skin self-regulates and stops producing vitamin D once it has made enough. Food will not cause it either.
Toxicity comes almost exclusively from large supplement doses taken over a long period, usually well beyond what was advised. The problem is not the vitamin D but the calcium it lets in: excess absorption leads to hypercalcemia, meaning too much calcium in the blood. Vitamin D is rarely interpreted alone, because it is one player in a system managing calcium.
When a result is low or confusing, your doctor will often order a calcium blood test, since calcium is what vitamin D protects. Because much of your calcium binds to the protein albumin, laboratories can also calculate a corrected calcium value that reflects the situation more accurately.
Your doctor may then request a parathyroid hormone test, because raised PTH alongside low vitamin D signals the body compensating at the skeleton’s expense. The workup often includes a phosphorus blood test, and clinicians frequently review an alkaline phosphatase test, an enzyme that rises when bone is actively remodelled.
Since PTH cannot work without magnesium, your doctor may check a magnesium blood test. Vitamin D is one of the most actively debated topics in medicine, and recent years changed the conversation more than the previous decade did.
The Endocrine Society withdrew its own thresholds
In a 2024 guideline communication, McCartney and colleagues explained that the Endocrine Society no longer endorses the definitions of “sufficiency” (at least 30 ng/mL) and “insufficiency” (20 to 30 ng/mL) it proposed in 2011. Those numbers came largely from observational studies, which watch what happens without intervening and can only show associations. When large randomised trials arrived, where chance decides who gets treatment or placebo, the panel found they did not support any threshold that reliably identifies who benefits.

The systematic review by Shah and colleagues underpinning that guideline searched for trials measuring the benefits and harms of screening healthy people with a 25-OH test. It found none. A systematic review is a structured search gathering every study meeting set quality criteria, so this is a meaningful absence rather than an oversight.
What this means for you: the value of testing a healthy person with no risk factors is genuinely unknown, neither proven nor disproven. That gap is why both the Endocrine Society and the US Preventive Services Task Force decline to endorse routine screening.
Who might benefit and who should be tested are different questions
The 2024 guideline led by Demay and colleagues drew a distinction that is easy to miss. The panel suggests vitamin D for certain groups, including children and adolescents, adults over 75, people who are pregnant, and people with high-risk prediabetes. Yet it still suggests against routine 25-OH testing, including in people with obesity or darker skin.
What this means for you: “might benefit from vitamin D” and “should have a vitamin D test” are separate questions, and answering one does not settle the other. Whether you fall into any of these groups is a discussion for your doctor.
More is not automatically better
A 2024 meta-analysis by de Souza and colleagues, which statistically combines results across randomised trials, examined healthy adults aged 60 and over without osteoporosis. Supplementation did not reduce overall fracture rates in this group, and the pooled trials showed a signal of more hip fractures among women taking it. The authors treat this as a caution about high intermittent doses given without knowing someone’s vitamin D level or calcium intake, not as a settled finding.
