Total urine protein includes albumin plus other proteins, and PCR is built the same way: protein divided by creatinine in a single sample. Creatinine clearance combines both measurements. Why muscle mass changes what your ratio means

Because creatinine comes from muscle, how much you excrete depends on how much muscle you carry. A muscular 25-year-old man may put out several times the creatinine of a small, frail woman of 85. Both are entirely normal for themselves. But creatinine is the denominator of the ratio, and changing a denominator changes the answer.
Work the arithmetic through. Suppose two people lose exactly the same amount of albumin into their urine each day. The muscular man has a large creatinine denominator, so his ACR reads lower. The frail older woman has a small denominator, so hers reads higher. Identical true albumin loss, two very different numbers on paper.

Shorter-term shifts push in the same direction. Intense exercise, a large meat meal in the hours before the sample, and creatine supplements all change how much creatinine turns up in urine. Timing matters more than most people expect. First-morning samples are preferred for ACR because they are more consistent than samples taken at random through the day, having sat in the bladder overnight without the effects of standing, walking and drinking layered on top.
Exercise shortly beforehand can transiently raise protein and albumin. Menstrual bleeding contaminates a sample.
