The CEA blood test measures carcinoembryonic antigen, a protein doctors use mainly to follow colorectal cancer over time rather than to go looking for it. After birth, production drops sharply, and a healthy adult keeps only a trace of it circulating in the blood.

The test itself is an ordinary blood draw, usually from a vein in the arm, with no fasting required. The laboratory reports the result in nanograms per milliliter (ng/mL), a measure of how much protein sits in a given volume of blood. Most laboratories treat anything up to roughly 5 ng/mL as normal for a non-smoker and set a slightly higher ceiling for smokers. CEA has three well-established jobs. All three involve someone who already has a colorectal cancer diagnosis.

A baseline before surgery

Before an operation to remove a colorectal tumor, the team measures CEA once to establish a starting point. This single number matters for two reasons. It gives the team something to compare every future result against, and it reveals whether your particular tumor produces CEA at all. A substantial share of colorectal tumors never raise it, and knowing that in advance changes how much weight your team will give the test for years afterwards.

Surveillance after treatment

Once a tumor has been removed, CEA should fall back into the normal range within about four to six weeks, because the protein clears from the blood within days. From then on, most follow-up programs repeat the test every few months for several years, alongside CT imaging and colonoscopy. Monitoring advanced disease

A falling trend alongside stable or shrinking scans is reassuring. A rising trend prompts a closer look with imaging. Why CEA is not a screening test

CEA is not used to look for cancer in people who have no symptoms and no diagnosis. The National Cancer Institute is explicit that markers of this kind are not suitable for screening the general population, and no US guideline recommends CEA for that purpose. How to read your CEA blood test result

A CEA result is almost meaningless on its own. Two habits turn it into useful information.

Why the trend beats a single number

Clinicians read CEA the way you would read a share price: the direction of travel, not today’s figure. A CEA of 7 ng/mL that has sat at 7 for three years is a very different message from a CEA of 7 that was 2 six months ago and 4 three months ago. The first pattern is a stable quirk of your biology. The second is a trend, and a trend is what triggers action.

This is why your first result after a diagnosis is rarely interpreted in isolation, and why a single mildly raised value in someone with no cancer history usually leads to a repeat test rather than a scan. One point is not a line. Two or three points, moving steadily in the same direction, are.

Why the same lab and the same assay matter

Different manufacturers make different CEA assays — the chemical kits laboratories use to measure the protein. They are calibrated differently, and the same blood sample can return meaningfully different numbers on two different systems. A jump from 3 to 6 ng/mL might reflect a real biological change, or it might reflect nothing more than your blood being sent to a different laboratory.

The consequence is simple: serial CEA measurements should be run on the same assay, at the same laboratory, wherever possible. Benign causes of a raised CEA — starting with smoking

Smoking is the single most common non-cancer reason for a raised CEA, and it deserves to be said first because it explains so many worrying results. Tobacco smoke irritates the lining of the airways, and those irritated cells release CEA into the blood. Current smokers therefore run a higher baseline than non-smokers — often in the 5 to 10 ng/mL band — with heavier smokers higher still.   • Liver disease such as cirrhosis or hepatitis, partly because a struggling liver clears CEA less efficiently.
  • Chronic obstructive pulmonary disease (COPD) and chronic bronchitis.
  • Benign colorectal polyps — growths that are not cancer.
  • Peptic ulcers, diverticulitis and some infections.
  • Kidney impairment, which slows the removal of CEA from the blood.

These are tendencies, not rules. Some cancers produce only a mild rise, and a few benign conditions a striking one. Only your doctor, holding your full history, can place your number in context.

What a rising CEA after treatment actually triggers

If you are in follow-up after colorectal cancer treatment and your CEA starts climbing, it is worth knowing what happens next — because the answer is rarely dramatic.

The first step is almost always confirmation. A single rise is repeated a few weeks later, on the same assay, to see whether it is real and still climbing. Many apparent rises simply do not repeat. If the rise is confirmed and sustained, the usual next move is imaging — typically a CT scan of the chest, abdomen and pelvis. A colonoscopy may follow, and a PET scan is sometimes used when standard imaging finds nothing but the marker keeps rising.

What a rising CEA does not do is diagnose a recurrence by itself. It is a prompt to look, not a verdict. And in a meaningful minority of cases, the looking finds nothing at all, the marker settles, and follow-up simply continues.

Why a normal CEA does not exclude colorectal cancer

This is the mirror image of the reassurance above, and it matters just as much.

A normal CEA does not rule out colorectal cancer. Some colorectal tumors — including some advanced ones — simply do not secrete CEA, so the marker stays flat while the disease is present. Poorly differentiated tumors, meaning those whose cells have lost their normal structure, are particularly likely to be CEA-silent. Early-stage tumors also often fail to raise it, which is another reason the test makes a poor screen.

The practical implication is important: if you have symptoms that concern you — a persistent change in bowel habit, blood in the stool, unexplained weight loss, ongoing abdominal pain or unexplained anemia and the blood tests used to investigate it — a normal CEA is not a reason to stop investigating. Other cancers that can raise CEA

Colorectal cancer is where CEA is most useful, but it is not the only cancer that can raise it. Cancers of the pancreas, stomach, esophagus, lung, breast, thyroid (specifically medullary thyroid cancer), ovary and liver can all push CEA up. Because so many different tumors can do this, a raised CEA never points to a particular organ on its own — it is a non-specific signal, which is precisely why it is used to follow a known diagnosis rather than to hunt for an unknown one.

When to see a doctor about your CEA blood test

Most people reading a mildly raised CEA do not need urgent care. They need a plan. Book a conversation with your doctor if any of the following apply:

  • Your CEA has risen across two or more consecutive tests, particularly if the tests were run at the same laboratory.
  • Your result is markedly high — well above your laboratory’s range — rather than just over the line.
  • You are in follow-up after colorectal cancer treatment and your level has climbed above its post-treatment baseline.
  • A raised result comes with symptoms: a lasting change in bowel habit, blood in the stool, unexplained weight loss, persistent abdominal pain or unusual fatigue.
  • You have no explanation for a raised result — you do not smoke and have no known bowel, liver, pancreatic or lung condition.

Equally, a stable mild elevation in a smoker, or in someone with known inflammatory bowel disease, is often exactly what the doctor expects. Bring previous results if you have them. The trend is the conversation.

Latest scientific advances in CEA testing

Research published since 2023 has sharpened, rather than overturned, how CEA is understood. Four findings stand out for patients.

First, on how much a raised CEA should worry you. A team at the University of Tokyo reviewed patients free of colorectal cancer recurrence for at least five years after surgery, who also had none of the usual explanations for a high CEA — no smoking, no bowel, liver or lung disease. Even in that filtered group, about one in five had a raised CEA at some point, and almost all those false alarms were mild rather than dramatic. What this means for you: an unexplained mild rise, with no other signal, is a recognized and common event rather than an ominous one.

Second, on how much CEA can be trusted to catch a recurrence. A systematic review and meta-analysis — a study that pools the results of many earlier studies — from a surgical team at Ohio State University compared CEA, imaging and circulating tumor DNA (fragments of tumor genetic material shed into the blood) as ways of detecting recurrence after colorectal surgery. CEA proved fairly specific but not especially sensitive: when it rises it usually means something, but many recurrences occur without it rising at all. PET scanning was the most sensitive strategy compared. What this means for you: CEA is a supporting instrument, and imaging remains the workhorse — which is why a normal CEA never replaces your scheduled scans.

Third, on why your own baseline matters. A team at Korea University followed patients whose colorectal cancer came back, splitting them by whether their CEA had been normal or high before surgery. Among those high beforehand, roughly six in ten showed a rise when the cancer returned. Among those normal beforehand, only about one in four did. What this means for you: if your tumor was a CEA producer before surgery, the marker is a useful tripwire for you personally. If it was not, your team will lean more on imaging — a deliberate choice, not an oversight.

Fourth, on what might come next. Work from the CIRCULATE-Japan GALAXY study tested a blood assay that looks for traces of tumor DNA after surgery. It predicted relapse considerably better than CEA did, often several months before trouble became clinically obvious. A large cohort study from Kurashiki Central Hospital in Japan — a cohort being a group of patients followed over time — pointed the same way, finding that CT scanning, not tumor markers, triggered detection of the great majority of recurrences. What this means for you: these DNA-based tests are still being evaluated and are not standard follow-up everywhere, but the direction of travel is towards combining several signals rather than leaning on one number. For now, CEA remains a cheap, widely available and genuinely useful part of that picture — read as a trend, alongside everything else.
