The Medicare annual wellness visit is one of the most searched benefits in Original Medicare, and one of the most misunderstood. Many people expect it to include a full physical exam and a round of blood work. According to Medicare.gov, it is “a conversation-based visit” that is not a routine physical exam, and it does not specifically list lab tests as part of the visit.

So which blood tests does Medicare actually cover, how often, and what might you pay? This guide separates the visit from the screenings that are covered on their own, so you know what to ask before your appointment.

What the Medicare annual wellness visit includes

The visit is designed to create or update a personalized prevention plan. Medicare.gov says it can include a health risk assessment questionnaire, routine measurements such as height, weight and blood pressure, a review of your medical and family history and current prescriptions, and a cognitive assessment for signs of dementia. You also receive a written checklist of recommended screenings, vaccines and other preventive services.

It is available once every 12 months. Your first visit cannot take place within 12 months of enrolling in Part B or of a “Welcome to Medicare” preventive visit. If your provider accepts assignment, you pay nothing for the visit itself, and the Part B deductible does not apply to it.

One catch matters for your wallet. If a routine physical exam or extra services that Medicare does not cover under this benefit are added during the same appointment, you may owe the full amount for them.

Blood tests Medicare covers on their own

Because the wellness visit does not itself list lab work, the blood tests you may want are covered through separate preventive benefits. The table below summarizes three of them, based on the corresponding Medicare.gov pages.

For diabetes screening, Medicare.gov lists the qualifying risk factors. You need at least one of these: high blood pressure, a history of abnormal cholesterol or triglyceride levels, obesity, or a history of high blood sugar. Or you need two or more of these: age 65 or older, being overweight, a family history of diabetes, or a history of gestational diabetes.

Other amounts can still apply depending on other insurance you hold, what your doctor charges and where the test is done.

Other lab tests ordered during the year

Outside these screenings, Medicare Part B covers clinical diagnostic lab tests that are medically necessary and ordered by your doctor. In Original Medicare you usually pay nothing for Medicare-approved tests, as long as the lab meets Medicare requirements. Latest scientific advances: why the lipid test earns its place

A global analysis published in August 2026 in JAMA, part of the Global Burden of Disease 2023 project, estimated how much heart disease and ischemic stroke is linked to high LDL cholesterol. It is a modeling study built on hundreds of studies and on randomized trials, not a trial run on participants.

Its authors describe LDL as a modifiable risk factor. Age-adjusted rates have fallen since 1990, yet the total number of cases keeps rising as populations grow and age, and measurement gaps remain. What this means for you: knowing your LDL is a useful reference point for a prevention conversation, but this study says nothing about your personal target. Your clinician sets that with you.

How to prepare for your visit

  • Bring your list of current medications and any recent lab results.
  • Write down family history of diabetes, heart disease or cancer.
  • Ask which preventive tests you are due for and how long ago your last cholesterol and glucose tests were.
  • Ask whether any service planned for the day falls outside the wellness benefit, so there are no surprises on the bill.
